Findings
Right elbow - craniocaudal and lateral radiographs are available for review.
There is a Y-shaped Salter-Harris type IV fracture of the humeral condyle, with fracture lines extending from the articular surface in the mid humeral condyle to both the medial epicondyle and the lateral supracondylar crest. There is mild to moderate cranioproximomedial displacement of the medial fragment, and mild to moderate cranioproximolateral displacement of the lateral fragment. The fracture margins are sharply margined, although the most distal margin in the humeral epiphysis appears smooth and rounded. There is a small, angular osseous fragment adjacent to the medial coronoid process of the ulna (1.5mm). The soft tissues are moderately thickened, centered at the level of the fractures.
Diagnosis
- Acute right distal humeral Salter-Harris type IV fractures affecting the medial and lateral aspects (Y-shaped) with associated soft tissue swelling/edema/hemorrhage. Incomplete ossification of the humeral condyle should be considered. Radiographs of the contralateral limb are recommended as this can be a bilateral finding.
