Findings
Orthogonal radiographs of the thorax are available for interpretation.
There are numerous, pinpoint, mineral opaque foci throughout the lungs. No soft tissue opaque pulmonary lesions are identified.
At the edge of the film, and most appreciated on the VD there is an expansile osteolytic lesion centered on the distal scapula, resulting in complete destruction of the acromion/distal scapular spine with additional lysis of the scapular neck. The soft tissues superficial to this lesion are moderately swollen. The remaining included skeletal structures and glenohumeral joint are normal. There is also an expansile lesion centered on the left 8th rib without evidence of destruction.
The cardiovascular structures and airways are normal. No abnormalities are detected in the pleural space. A minimal amount of gas is detected in the cranial thoracic trachea, considered incidental. An incidental ballistic projectile is detected in the subcutaneous tissues along the caudodorsal neck. There is moderate multifocal spondylosis deformans along the thoracic spine.
Diagnosis
- Expansile osteolytic lesion of the left scapula is most concerning for malignant neoplasia, such as a primary bone tumor or other sarcoma.
- Expansile lesion affecting the left 8th rib, also likely to be neoplastic in origin.
- Benign pulmonary osseous metaplasia. No evidence of pulmonary metastatic disease.
Discussion
FNAs of the scapula were performed and were most consistent with an osteosarcoma
