1 year old mixed breed dog male entire. Occasional coughing episodes

Findings

Opposite lateral and VD radiographs of the thorax are available for interpretation.

The margins of the right diaphragmatic crus are not identified. There are numerous gas-filled tubular structures and a portion of the colon and stomach filling the right hemithorax and a portion of the mid-left hemithorax, resulting in a leftward mediastinal shift of the cardiac silhouette. The cardiac silhouette is also displaced dorsally. The splenic tail is identified in the cranioventral thorax on the lateral projections. The unobscured margins of the cardiac silhouette are normal. The lungs have a mild diffuse unstructured interstitial pattern and are being displaced caudodorsally bilaterally. The pulmonary vasculature is normal. The caudal hepatic margins are seen at the level of the costal arches. The serosal detail of the included abdomen is normal.

Diagnosis

Right-sided diaphragmatic hernia with herniation of the small intestines, spleen and a portion of the colon and stomach +/- a portion of the liver. Severe compression of the lung lobes bilaterally with subsequent atelectasis.

Discussion

The patient underwent surgical hernia repair during which a splenectomy was performed. The patient was also neutered at the same time.
