Findings
Opposite lateral and VD radiographs of the abdomen were acquired.
The serosal detail is mildly decreased in the mid to caudal abdomen due to visceral crowding.
The abdomen is filled with an enlarged uterus containing two fully mineralized feti. One of the feti is positioned in the pelvic canal, hind end first, with its caudal aspect extending into the vaginal vault. The caudal third of this fetus (including the pelvis and pelvic limbs) is absent.
The stomach and a few segments of small intestines contain a small amount of gas. The colon contains a large volume of gas. The liver is normal. The unobscured margins of the spleen, kidneys and urinary bladder are normal.
The mammae are markedly enlarged, consistent with the patient’s status of pregnancy. The caudal vena cava is small.
Diagnosis
- Late term gestation with two feti; the described abnormalities of the fetus in the pelvic canal/vaginal vault is most suggestive of fetal mutilation by the patient.
- Evidence of hypovolemia/dehydration.
