History

3 month old male mixed breed dog. Fell of the porch two days prior

Findings

Orthogonal views of the right elbow have been performed.

There is a complete fracture extending through the distal humerus from the level of the mid aspect of the humeral condyle, through the distal humeral physis, and through the lateral aspect of the humeral metaphysis and diaphysis. There is mild lateral and proximal deviation of the fracture.

There is a moderate amount of intra-and extracapsular soft tissue swelling surrounding the elbow. There are open physes consistent with the patient’s young age.

Diagnosis

Salter-Harris type IV fracture of the right distal humerus with deviation as described. The surrounding soft tissue swelling is consistent with hemorrhage and edema.

Discussion

Surgical reduction and stabilization was performed. As this pathology may be due to underlying incomplete ossification of the humeral condyle, radiographs of the contralateral limb should be considered.
