Findings

Orthogonal radiographs of the abdomen are available for interpretation.

The clinically reported perineal swelling is identified in all imaged, but best appreciated in the left lateral radiograph. A urinary catheter is present with its distal end residing at the level of the peritoneal swelling , consistent with herniation of the urinary bladder. The urinary bladder is not identified within the abdominal cavity. There are striated gas foci extending through the pelvic canal with a round gas focus within the perineal swelling in the plane of the rectum. The intestinal tract is congregated in the caudal abdomen, but remains otherwise normal with minimal heterogeneous soft tissue opaque contents. There is no evidence of intestinal distension. The stomach contains gas and fluid. The serosal detail and remaining intraabdominal structures are normal.

Diagnosis

Discussion

The patient underwent surgery and has recovered well. Surgically there was a bilateral perineal hernia, worse on the right side, with herniation of the bladder, prostate and small intestines
