History
13-year-old mixed breed dog. Tachypnea. Diuretics were administered earlier.

Findings
Study: Opposite lateral and VD radiographs of the thorax were acquired
Findings: There is moderate to severe generalized cardiomegaly with elongation, widening and rounding of the cardiac silhouette and left atrial enlargement. The unobscured arteries and veins are normal to small. There is a multifocal, severe unstructured interstitial to patchy alveolar pattern throughout the pulmonary parenchyma, most severe in the perihilar region and cranioventrally. There is the impression of the cranial lobar pulmonary bronchi remaining widened in the periphery. The pleural space is normal. The thoracolumbar spine is undulating with multifocal narrowing of several intervertebral disc spaces, endplate sclerosis and spondylosis. The serosal detail in the cranial abdomen is normal.
Diagnosis
- Moderate to severe generalized cardiomegaly with the described pulmonary pattern is most concerning for cardiac disease (such as chronic degenerative mitral and tricuspid valve disease) and secondary left-sided heart failure. Absence of venous distension is not considered unusual, given the reported furosemide administration. An echocardiogram is recommended for further evaluation.
- Suspect peripheral bronchial dilatation may be exacerbated by the patient’s reported current respiratory distress, however, concurrent lower airway disease (non-infectious vs. infectious) remains possible.
- Thoracolumbar intervertebral disc disease.
