Findings
Study: Orthogonal whole body radiographs are available for interpretation.
Findings: The serosal detail is generally decreased. There is a large, lobular, soft tissue opaque mass in the mid to ventral abdomen, caudal to the stomach. This mass is filling a large portion of the abdomen and is causing craniodorsal and caudal displacement of the intestinal tract. On the VD projection, there is the impression that the mass extends along the lateral aspects of the abdomen, resulting in medial displacement of the intestinal tract toward midline. The intestinal tract contains a small to moderate volume of gas. The unobscured margins of the liver and kidneys are normal. There are multiple, fusiform to lobular soft tissue opacities in the perineal and inguinal region, consistent with normal male reproductive structures.
The thoracic structures are normal.
There is a large, rectangular osseous body at the proximal most aspect of the left olecranon. No soft tissue swelling is present at this level.
Diagnosis
- Large, soft tissue opaque mass(es) in the mid-ventral abdomen may represent enlarged lymph nodes, be associated with the gastrointestinal tract or spleen or may be a non-organ associated mass(es). Decreased serosal detail is likely in part due to mass-effect and visceral crowding, however, concurrent peritoneal effusion is possible. An abdominal ultrasound and tissue sampling are recommended for further evaluation.
- Normal thorax with no overt evidence of metastatic disease.
- Non-union fracture of the proximal aspect of the left olecranon.
Discussion
The patient underwent abdominal ultrasound which confirmed the splemomegaly. FNAs were consistent with lymphoma.
