Findings
Dorsopalmar and lateral radiographs of the right manus.
There is complete lysis of the mid body of the distal phalanx of the second digit. There is marked moth-eaten lysis of the ungual process, ungual crest and flexor tubercle of the same phalanx with decreased definition of the distal interphalangeal joint space. There is smooth osseous proliferation of the medial aspect of the proximal interphalangeal joint. There is a small ovoid mineral focus at the lateral aspect of the mid body of the middle phalanx. There is moderate soft tissue swelling surrounding the distal interphalangeal joint.
Diagnosis
- Monostotic aggressive lysis affecting the distal phalanx of digit 2 on the right side and associated soft tissue swelling. This is most consistent with a neoplastic process, with primary consideration given to melanoma especially given the patient’s history. Squamous cell carcinoma cannot be ruled out.
- The remainder of the ossesous changes are likely degenerative in nature.
Discussion
The patient was later on euthanized due to his confirmed melanomal to the lip. No definitive confirmation was obtained for the digital lesion.
