Findings
Opposite lateral and VD radiographs of the thorax were acquired by the emergency service.
There is mild to moderate enlargement of the main pulmonary artery. The pulmonary arteries are moderately enlarged, tortuous and blunted, most severe of the left caudal lobar artery. There is mild right-sided heart enlargement characterized by increased sternal contact, widening of the cardiac silhouette and rounding of the right side of the heart on the VD view.
There is a moderate, diffuse bronchointerstitial pattern.
There is a broad-based, soft tissue structure dorsal to the 1st and 2nd sternebrae, in the region of the sternal lymph node. The pleural space is normal.
There is mild osteophytosis of the glenohumeral joint bilaterally. There is incidental spondylosis deformans. The included cranial abdominal structures are normal.
Diagnosis
- Right-sided cardiomegaly, main pulmonary artery enlargement, tortuous and enlarged pulmonary arteries, and diffuse bronchointerstitial pattern are most suggestive of pulmonary hypertension. Primary differential is heartworm disease.
- Sternal lymphadenopathy may be reactive and is suggestive of abdominal pathology.
