Did you notice how the fracture extends into the tibio-tarsal joint ?

Findings

Orthogonal views of the right crus have been performed.

There is a long oblique fracture of the mid diaphysis of the right tibia is again identified and there is lateral, cranial, and proximal displacement of the fracture.

Extending distally from the distal fracture margin in a spiral fashion, there are long, radiolucent fissure lines. One of these fissure lines extends into the distal articular margin of the tibia at the level of the medial cochlea.

There is a complete, short oblique fracture of the mid diaphysis of the right fibula, with medial and proximal displacement.

There is also an incomplete short oblique fracture of the proximal diaphysis of the right fibula with a fissure line extending distally. There is focal caudal deviation of the fibula at the level of the fracture.

There is marked soft tissue swelling surrounding the right crus.

Diagnosis

Long oblique fracture of the mid diaphysis of the right tibia with displacement as described, with spiral fissure lines extending through the distal fracture fragment into the distal articular margin of the tibia.

Short oblique fracture of the mid diaphysis of the right fibula with displacement, and an incomplete short oblique fracture of the proximal diaphysis of the right fibula.
