History

6 year old male castrated cat. Acutely very lethargic

Findings

Orthogonal radiographs of the abdomen are available for interpretation.

There is fluid streaking within the retroperitoneum. The kidneys are severely enlarged (up to 4 times the length of L2) with lobular margins. The kidneys are causing ventral and rightward deviation of the colon and small intestines.

The liver, spleen, GI tract, and urinary bladder are normal. The patient has a large amount of intra-abdominal fat.

Diagnosis

Marked bilateral renomegaly, with lobular margins may represent a neoplastic process (such as lymphoma), or may represent polycystic kidney disease. Retroperitoneal changes may represent effusion, inflammation, or neoplastic infiltration.

Discussion

On abdominal ultrasound the renal changes were most suggestive of lymphoma. This was confirmed on FNAs of the renal parenchyma. The patient was subsequently euthanized.
