Findings

Opposite lateral and VD projections of the abdomen acquired by the emergency service are available for interpretation.

A wet hair artifact is noted along the ventral abdomen.

Numerous small gas opacities are noted within the dorsal abdominal and lumbar subcutaneous tissues as well as along the left abdominal wall and in the inguinal region. Several gas opacities are superimposed with the abdominal cavity. There is a fracture of the distal aspect of the left 12th rib.

There is thickening of the left abdominal wall. A fairly well-defined, triangular soft tissue opacity is noted in the cranial dorsal abdominal subcutaneous tissues, dorsal to the T12-L2 vertebrae, and best seen on the left lateral view. The splenic head is not seen within the left cranial abdomen on both the lateral and VD views. The spleen is normal in size and is sharply marginated.

The left kidney is normal. The right kidney is obscured. The urinary bladder is small and is of homogenous soft tissue opacity. The stomach is mildly gas distended. The small intestine is normal in size and distribution. The colon contains several pinpoint to small variably shaped mineral opacities and is otherwise empty.

Diagnosis

Extensive subcutaneous emphysema and left 12th rib fracture are consistent with the reported trauma. Triangular soft tissue opacity in the dorsal abdominal subcutaneous tissues is most consistent with the spleen, likely herniated through the abdominal wall.
