Findings
There is generalized enlargement of the cardiac silhouette, with dorsal deviation of the trachea. The pulmonary veins and arteries are equal in size, but are larger than normal. There is mild undulation of the right caudal lobar vein.
There is a diffuse, moderate bronchial and coalescing unstructured interstitial pattern. The pleural spaces normal.
The serosal detail in the cranial abdomen is normal.
The liver is mildly enlarged, extending past the costal arch, with rounded margins.
Diagnosis
- Generalized cardiomegaly, with enlarged pulmonary vessels is consistent with cardiomyopathy (such as hypertrophic cardiomyopathy). Givent the tortuous pulmonary vessels, pulmonary hypertension is also considered.
- The pulmonary changes are most suggestive of concurrent left sided heart failure. An echocardiogram may be considered for further evaluation.
- Hepatomegaly is nonspecific, and may represent benign (such as vacuolar hepatopathy or nodular hyperplasia) or malignant (such as infiltrative neoplasia) etiologies
Echocardiography
On echocardiogram, the cat was diagnosed with dilated cardiomyopathy and treatment was started.
