Findings
Orthogonal radiographs of the abdomen are available for interpretation.
The serosal detail is normal.
The spleen is diffusely markedly enlarged with lobular margination, and resulting in caudodorsal displacement of the left kidney.
The margins of the liver are partially superimposed with the enlarged spleen, however, the liver is not overtly enlarged.
The stomach contains a small volume of gas. The small intestines are normal in diameter and contain a mixture of fragmented gas and homogenous soft tissue opaque material. The ascending and descending colon are moderately distended with gas, and the transverse colon is not clearly seen.
There is mineralization within the plane the kidneys. The urinary bladder is normal.
The included musculoskeletal structures are normal.
Diagnosis
- Marked splenomegaly is most indicative of an infiltrative neoplastic disease, such as lymphoma or mast cell tumor.
- The appearance of the intestinal tract is suggestive of inflammatory bowel disease; however, intestinal lymphoma cannot be ruled out.
- Renal mineralization.
Discussion
Abdominal ultrasound was performed:
The spleen is markedly diffusely enlarged and hypoechoic. It contains few, small, ill-defined hypoechoic nodules throughout its parenchyma. A few strongly hyperechoic, perivascular and subcapsular nodules are also present and consistent with incidental myelolipomas.
FNAs were obtained and were conclusive for lymphoma. The cat was started on chemotherapy.
