11 year old male neutered cat. Anorexia

Findings

Orthogonal radiographs of the abdomen are available for interpretation.

The serosal detail is normal.

There is a large mass (approximately 6.0cm in length and 4.5cm in diameter) with lobular margination in the right mid ventral abdomen. The cranial aspect of the mass is smooth and outlined by gas in the ascending colon, which extends aborad from this mass and is moderately distended with gas. Within the caudoventral aspect of this mass, there is a small gas focus.

The liver and spleen are diffusely mildly enlarged.

The kidneys measure at the upper limits of normal size (approximately 2.8x the length of L2 on the VD view) (illustrated on the VD radiograph). The urinary bladder is normal. There is an incidental multifocal spondylosis deformans.

Diagnosis

Intestinal mass at the ileocolic junction, with hepato-splenomegaly most concerning for a disseminated neoplastic process, particularly lymphoma or mast cell tumor. Alternatively, the intestinal mass may also reprsent an adenocarcinoma, and the other findings may represent benign processes, such as vacuolar hepatopathy, and passive splenic congestion from sedation. The small gas focus at the caudoventral aspect of the mass could represent intra luminal gas or region of necrosis/abscessation.

Discussion

FNAs of the mass were performed with ultrasound but were inconclusive. The owner elected to not pursue further diagnosis.
