History

13 year old mix breed dog. History of B2 heart disease. Coughing that led to syncopy. Tachypneic

Findings

The left atrium is mildly enlarged. The pulmonary veins are larger compared to the corresponding arteries.

There is a marked unstructured interstitial pattern within the caudal dorsal lung lobes, worse on the right side.

There is mild bilateral elbow DJD, a small ovoid mineral opacity just caudal to one of the medial epicondyles, and mild spondylosis deformans of the thoracolumbar spine.

The included cranial abdominal structures remain within normal limits.

Diagnosis

The pulmonary and vascular changes are consistent with pulmonary edema secondary to left-sided heart failure. Mild left atrial enlargement is consistent with the reported mitral valve disease

Static mild bilateral elbow DJD. The small mineral opacity adjacent to the medial epicondyle may represent flexor enthesopathy, separate center of ossification of the medial epicondyle, or soft tissue dystrophic mineralization.

Discussion

An echocardiogram was performed confirming mitral and tricuspid valve endocardiosis as well as mild pulmonary hypertension. The patient was started on Pimobendan, Enalepril and Furosemide.
