History

2 months old mixed breed dog. Increased respiratory rate.

Findings

Opposite lateral and VD projections of the thorax are available for interpretation.

The physes are open, consistent with the patient's young age.

Numerous small intestinal segments, a portion of the ascending and transverse colon as well as fat are noted within the thorax, summating with the cardiac silhouette.

This is associated with a globoid cardiac silhouette, dorsal displacement of the trachea and compression of the caudal lobar mainstem bronchi.

A small intestinal segment as well as a portion of the colon can be seen extending from the peritoneal cavity into the thoracic cavity.

The cardiac outline is obscured by the superimposed structures.

A mild diffuse unstructured interstitital pattern is noted in the visible portion of the lungs.

Diagnosis

Peritoneopericardial diaphragmatic hernia with small intestines, large intestines and fat within the pericardial sac. Although the majority of the liver appears to be within the peritoneal cavity, the presence of a portion of the liver within the pericardial sac cannot be entirely ruled out. Pulmonary changes are most likely due to atelectasis.
