History

7 year old male castrated cat. Urethral obstruction relieved by passing a urinary catheter.

Findings

Orthogonal projections of the abdomen are available for interpretation.

The serosal margin detail is diffusely decreased within the peritoneal cavity, with marked fluid streaking of the peritoneal fat. The urinary bladder is not clearly identified. A radiopaque urinary catheter is seen within the plane of the urinary bladder and urethra with its distal tip in the plane of the urinary bladder. A small gas opacity is noted just cranial to the tip of the urinary catheter on the lateral views.

The liver is normal. The spleen is mildly diffusely enlarged. The kidneys are normal. The stomach contains a small amount of gas and is minimally distended. The small intestine is mostly obscured but the visible portion is normal. The colon contains a small amount of formed fecal material.

A positive contrast cystogram was performed under fluoroscopic guidance.

Approximately 15 ml of diluted nonionic iodinated contrast medium was injected into the urinary bladder under fluoroscopic guidance. Following mild distension of the urinary bladder with contrast medium, a thin stream of contrast medium was seen leaking into the peritoneal cavity from the level of the cranial bladder wall ventrally.

Diagnosis

Urinary bladder rupture and secondary uroabdomen following previous urethral catheterization. The integrity of the urethra was not evaluated.

Mild splenomegaly is nonspecific and may be secondary to sedation, may represent a benign process (such as extramedullary hematopoiesis or lymphoid hyperplasia) or may represent diffuse neoplastic infiltration.
