Findings
Orthogonal radiographs of the abdomen are available for interpretation
The abdomen is pendulous and there is poor serosal detail with fluid streaking, primarily in the peritoneal space but also in the retroperitoneal space.
The left kidney is markedly enlarged measuring about twice as much thant the right kidney. Overall its margins are rounded and it remains diffusely soft tissue opaque. The majority of the intestines are displaced to the right and ventrally by the left kidney. The right kidney is partially obscured but is considered normal. The urinary bladder margins are also obscured.
The liver is mildly enlarged overall and is subjectively larger than in the previous thoracic study. The spleen is normal.
Diagnosis
Severe left renomegaly. Primary consideration is given to a neoplastic process such as carcinoma or lymphoma. Unilateral hydronephrosis cannot be ruled out.
Peritoneal, pleural, and probable retroperitoneal effusion. This is most likely secondary to hypoproteinemia given the history of protein losing enteropathy. The retroperitoneal effusion could also be paraneoplastic or hemorrhage from the large left kidney mass.
