History

10 year male castrated old Cavalier King Charles. Tachypneic, has worsened over the last week.

Findings

There is severe left atrial, left ventricular, right ventricular and right atrial enlargement. The pulmonary veins are larger than the corresponding pulmonary arteries, most evident in the cranial ventral lung fields on the right lateral view. There is a severe perihilar alveolar pattern.

The liver extends beyond the costal arch and has rounded margins. The caudal vena cava is difficult to individualize on the right lateral projection but does not appear to be enlarged on the left lateral projection.There is no evidence of abdominal effusion in the visible abdomen.

Diagnosis

Severe generalized cardiomegaly and congestive left heart failure.

Hepatomegaly may indicate underlying hepatic disease or be related to right sided heart failure.

In a dog of this age, mitral and tricuspid regurgitation is the most likely underlying etiology. Echocardiography is recommended to further assess.
