History

15 year old Shi-Tzu, vomiting and lethargic for 3 days. Unremarkable bloodwork. Previous history of nephroliths and uroliths.

Findings

Orthogonal radiographs of the abdomen are available for interpretation.

Most noted on the lateral views there is a moderately gas distended intestinal segment within the caudal abdomen which measures twice the height of the L2 vertebra, this is not identified on the VD view. This described segment is located ventrally to the colon on the left lateral view. The remaining small intestines are moderately fluid and minimally gas filled without overt distention. A soft tissue opaque mass is visible within the right inguinal region. Within that location very faint tubular gas opacities are visible on the right lateral view. Two round gas bubbles are noted within the same location on the VD view superimposed over the right iliac wing.

Mild hepatomegaly with multiple identified mineralized punctate foci within the plane of the liver. Incidental linear metallic foreign object at the caudal liver margin. Multiple calculi are visible within the left renal pelvis and faint linear mineralized foci are seen within the plane of the right kidney. There is a focal lucency within the mid aspect of the penis.

Diagnosis

Right inguinal hernia with small intestinal herniation with secondary strangulation. Ischemia was also observed on color doppler (images not provided).

Nephrolithiasis and unspecific hepatomegaly. The focal lucency in the os penis could represent pressure atrophy secondary to previous uretrolith.
