History

6 year old male chihuahua diagnosed with benign prostatic hyperplasia by rDVM and neutered in January 2015. Presenting for difficulty urinating and defecating.

Findings

The prostate is markedly enlarged and is deviating and compressing the distal descending colon/rectum dorsally. A large amount of soft tissue opacity is present in the caudal retroperitoneal region, displacing and distal descending colon ventrally. Adjacent to this region, there is a large amount of irregular osseous proliferation along the ventral aspects of L5, L6, and L7, as well as along the ventral and medial aspect of the left ilium. On the ventrodorsal projection, a large region of mineral opacity is noted overlying the left transverse process of L7. No abnormalities of the remaining abdominal organs are appreciated. Serosal detail and the detail in the cranial retroperitoneal region are within normal limits. Numerous, variably sized, well-defined soft tissue nodules are present in the thorax (up to 0.7 cm). Incidental findings include a bipartite os penis, a metallic BB in the soft tissues ventrolateral to the left coxofemoral joint, and collapse of the intervertebral disc space at T11-T12 with ventral spondylosis.

Impression: Marked prostatomegaly most consistent with prostatic carcinoma. Large amount of soft tissue opacity in the caudal retroperitoneal region, most consistent with marked sublumbar lymphadenopathy. Irregular osseous proliferation of the caudal lumbar vertebra and left ilium. Pulmonary nodules most consistent with metastatic disease.

Diagnosis

Presumptive prostatic carcinoma with metastasis to the sublumbar lymph nodes, caudal lumbar vertebra, left ilium, and lungs.
