Findings

In both projections, the cardiac silhouette is not visualized due to large amounts of pleural fluid (900mL of fluid had been pulled off prior to radiographs).

There is soft tissue opacity in caudoventral thorax effacing margins of the diaphragm. Additionally, ill-defined soft tissue opacity is seen in the regions of the hilar lymph nodes.

Gas opacities superimposed over increased opacity in the lung representing free air secondary to pleural tap.

Diagnosis

1) Caudoventral soft tissue opacity most concerning for a primary lung tumor involving the accessory lung lobe. A diaphragmatic hernia cannot be definitively ruled out based on the border effacement of the diaphragm. However, based on abdominal radiographs (not included on this case of the week), and correct anatomical location of the viscera, it made this differential less likely.

Mass was confirmed on thoracic ultrasound.

2) Large amount of pleural fluid, likely a neoplastic effusion 3) Concern for perihilar lymphadenopathy.

Pathology Report

Histiocytic sarcoma of the pulmonary tissue.

Pleural fluid:protein poor transdute, concerns for neoplastic effusion.
